When you submit the Start Here form on this website, Fitness Artist may collect the information you provide, including:

• First name and last name
• Email address
• Phone number
• The program you are most interested in
• Whether you expect to use Child Watch
• Your goals, and any optional notes about what has made fitness difficult

To help us understand how you arrived at the site, the form may also collect attribution information such as the page URL you submitted from, a referring website, and UTM parameters (source, medium, campaign) when they are present in the link you used.

A submission timestamp and a unique lead identifier are also stored with your information.

Separate from the Start Here form, Fitness Artist uses a Participant Intake that is completed before someone begins training with us. It is a different form with a different purpose, and it collects more information. Depending on whether the participant is an adult or a minor athlete, the intake may collect:

• Participant name, date of birth and age, email, and phone number
• Parent or legal guardian name, email, and phone number for a minor athlete
• Emergency contact name and phone number
• The program or activity the participant will take part in, and any team or organization
• Limited health and readiness information relevant to safe coaching and emergency response, such as current injuries or physical limitations, restrictions given by a physician, recent surgery or significant injury, relevant health conditions, allergies and emergency medications for a minor athlete, and medications or health circumstances that may materially affect safe exercise
• Any additional notes the participant or guardian chooses to share
• Optional permission to use photos or video
• An electronic signature, the participation agreement accepted, its version, and the date and time of acceptance
• Basic technical and operational information such as the browser user agent, the page the intake was submitted from, a unique intake identifier, and a record of which staff member viewed or updated the record

Fitness Artist uses Participant Intake information to:

• Prepare coaches to train each participant safely
• Identify information that our staff should review before someone participates
• Respond appropriately in an emergency
• Administer and keep a record of consent and participation agreements
• Operate and secure the intake system itself

Fitness Artist coaches are not using this form to diagnose, treat, or manage any medical condition. If the intake marks a submission for staff review, that is an internal coaching and safety step. It is not a medical clearance, a diagnosis, or an assessment of fitness to exercise. Questions about whether exercise is appropriate for you belong with your physician.

Intake records are stored in our Supabase database. When an intake is submitted, our team receives an operational notice through Make and email so we know to follow up. That notice is limited to the participant name, whether the participant is an adult or minor, the guardian name where relevant, the program, the submission time, an intake identifier, and a link for staff to sign in. It does not contain health answers, screening or review status, or the signature.

Health and readiness answers, emergency information, signatures, and staff notes are restricted to authorized Fitness Artist personnel with a business need to know. Access requires a signed-in staff account that has been explicitly granted intake review permission, and individual record access is recorded in an internal access history. Health answers and signatures are not sent to analytics tools and are not included in the operational notice described above.

Fitness Artist uses reasonable administrative and technical safeguards for this information. We do not claim any specific security certification, and we do not describe this system as HIPAA compliant.

A parent or legal guardian must complete and authorize the intake for a minor athlete. Photo and video permission is optional and can be declined without affecting participation.

You may contact us to ask what we hold, correct information, or request deletion of an intake record. We honor those requests subject to legal, safety, insurance, recordkeeping, and other legitimate business obligations, which can mean that a signed agreement or incident related record is retained for a period of time. Fitness Artist uses the information you submit to respond to your inquiry, understand which services you may be interested in, and communicate with you about next steps. We also use it to operate and improve the inquiry process on this website.

The Start Here form is intended to start a conversation, not to enroll you in a marketing list. We do not use the information for advertising, profiling, automated decision-making, newsletters, or SMS marketing.

Form submissions are stored and processed using third-party service providers that support this website and our inquiry workflow. This project currently uses Supabase to store submitted information and Make to route a notification to our team so we can follow up.

These providers process information on our behalf to help operate the site and respond to inquiries. We rely on them to support secure operation, and we do not make unsupported claims about their internal certifications, retention practices, or infrastructure beyond what they actually provide to this project.

Information you submit may be shared with service providers that help operate this website and process inquiries, and when required to do so by law.

Fitness Artist does not sell personal information.

Fitness Artist uses reasonable administrative and technical measures and relies on service providers to support secure operation of this website. However, no method of internet transmission or electronic storage can be guaranteed to be completely secure.

You may contact Fitness Artist to ask questions about the information you submitted, or to request that it be corrected or deleted, subject to legitimate business and legal needs. To do so, email info@fitness-artist.com.

This public website and the Start Here form are not directed to children under 13, and we do not ask children to submit information to us directly. (This is separate from Child Watch, which is a member benefit during select eligible sessions.)

Where a minor athlete trains with us, the Participant Intake described above is completed and authorized by a parent or legal guardian, not by the child.

Fitness Artist uses Google Analytics to understand how this website is used and how it performs. Google Analytics may collect information such as the pages viewed, browser and device information, approximate location derived by the service, referral or source information, and interactions such as a successful inquiry submission.

Fitness Artist does not intentionally send the personal information entered into the Start Here form (such as your name, email address, phone number, program interest, goals, or obstacles) to Google Analytics.

Information collected through Google Analytics is processed by Google subject to Google’s own terms and privacy practices. If additional analytics or advertising technologies are introduced in the future, this policy may be updated to describe them clearly.

Fitness Artist may update this Privacy Policy from time to time. When we do, we will revise the effective date at the top of this page. We encourage you to review this policy periodically.
